WardenNet tracks signatures on a per-patch basis. This means that every new patch will indicate new Warden signatures, even if the "new" Warden is identical to the last Wardens from the previous patch.
The range of time is based from the current second, so 0-4 hours ago really means 0-4 hours ago, not just "12pm to 4pm" or whatever time it happens to be, and 0-1 days ago really means 0 to 1 days ago, not just "March 30th, 2006" or whatever today happens to be. This means that if you check back in a few hours, you will see the 14-day history slowly shift to the bottom, it doesnt just change at midnight (that would be hard to read anyway, given that not everyone is in the Eastern Standard Time/GMT-5 zone).
The numbers given are the number of signatures that match the given heading (new, old, or total) for the given time span.
The new/old calculation is based on a point in time 24 hours before the beginning of the given range (even for the 48-hour history).
Any time span that has 5 or more total signatures seen may receive special coloring on the % of Total to indicate a high likelihood of a Warden update, or a recent Warden update, for that period. If the % of Total is greater than or equal to 60%, the % will be bolded. If the % of Total is greater than or equal to 75%, the % will be colored red, in addition to the bold.
